When uploading or turning in a physical please be sure to submit the WIAA Medical Eligibility Form or the WIAA Clearance Form.
We cannot accept a Well Child visit. Physicals for Athletics must be on the WIAA Medical Eligibility form or the WIAA Clearance form. There must be a Dr.'s signature, a date of the examination and somewhere on the physical it must indicate that the student/athlete is cleared for all sports without restriction, or cleared with the following restrictions and those restrictions indicated on the form.

**Please note: physical examination dates April 1 and thereafter is valid for the following two years; physical examination dates before April 1 is valid only for the remainder of that school year and the following school year.

Physicals must be dated on or after 04/01/2024 to be valid for the 2025-26 school year.

Letter from the MHS Athletic Director
First and foremost, thank you for supporting the athletic programs we offer at Eau Claire Memorial. As many of you know, we have a proud tradition of athletic competition in our history. As a Division One team in the Wisconsin Interscholastic Athletics Association (WIAA) and a member of the Big Rivers Conference, our accomplishments and successes have been embraced from one sport to the next.
But, with that said, we realize that sports is more than just a game. We continue to aim for excellence in the classroom as academics is our number one priority. In addition, we strive to teach our student-athletes important life lessons such as, but not limited to, teamwork, self-discipline, time management, cooperation, commitment, sportsmanship, perseverance, and sacrifice. Through participation in athletics, we provide our student - athletes an opportunity to grow and mature as young adults preparing them for life after high school.
Again, thanks for your encouragement and support as we move forward with the upcoming season.
